New nurse practitioner coming to Omemee Medical Centre
Chantelle Hart joins family health team, expanding access for local patients
The City of Kawartha Lakes Family Health Team is pleased to announce that Chantelle Hart, nurse practitioner, will be starting to see patients at the Omemee Medical Centre starting April 7, 2026.
Nurse practitioners are health professionals whose scope of practice includes wellness care, diagnosing illness, prescribing medications, ordering and interpreting diagnostic tests, referring patients to specialists, and performing procedures.
“I grew up in a small town and appreciate the unique qualities of small towns. I also understand how important it is to have accessible healthcare in a small town,” said Hart. “I have previously worked in medical clinics in rural Ontario and really enjoyed working in a rural setting. I am looking forward to caring for patients here,” she added.
Hart brings more than 20 years of experience to her practice in Omemee, including clinical roles at the Peterborough Family Health Team and St. Joseph’s at Fleming Long-Term Care Home. Her clinical focus includes managing diseases, older adult and children’s care, and women’s health. Hart has also taught at Queen’s University.
Mike Perry, executive director of the City of Kawartha Lakes Family Health Team, said Hart brings “the training and dedication our patients deserve, and her arrival strengthens our ability to deliver team-based care at the highest standard.”
“Every new health care professional we add helps real people here locally — our neighbours, families, seniors — who now have access to the care closer to home. We are grateful Chantelle has chosen to come to Omemee, and are pleased to have her apart of the health team,” Perry added.
Dr. Ruth Wilson, president of the Family Health Team, underscored the clinical significance for health care.
“Nurse practitioners are an essential part of modern, high-functioning primary care.” Wilson says Hart will be working in collaboration with Dr. Jane Clay, the nurse on site, Courtney Sitler, and Arlene O’Brien, medical receptionist.
Starting next week, existing Omemee patients who saw the former nurse practitioner can contact the medical centre at 705-799-0000 to see Hart. Clay continues to see her regular patients. Residents without a family doctor or nurse practitioner must first register with Health Care Connect by calling: 1-800-445-1822.
Funded by the Government of Ontario, the City of Kawartha Lakes Family Health Team has some 40 health care professionals – including nurse practitioners, registered nurses, and registered practical nurses, pharmacists, dietitians, social workers, and medical receptionists – working together with local family doctors to provide high-quality, focused care for more than 25,000 patients at locations across the City.
