Kawartha Lakes Haliburton Ontario Health Team launches new health and wellbeing service navigation platform
The Kawartha Lakes Haliburton Ontario Health Team (KLH-OHT) is pleased to announce the launch of a new online health and wellbeing service navigation platform. This new platform has been designed with our communities in mind and allows users to easily search for local programs and services offered by providers in Kawartha Lakes and Haliburton County. When users visit the website, they can easily find available services close to home and in some cases can self-refer or book an appointment directly through the platform.
The KLH-OHT is also excited to introduce its new navigator function that allows residents to speak directly with a representative about their health service navigation questions. If residents have questions about accessing health services, they can contact the team at (705)-934-1439.
This online navigation platform focuses on enhancing access to programs and services for older adults, as well as mental health and addictions services, and child and youth services. It also serves as a means of supporting referrals between local service providers so that the team can work together to ensure residents have access to all available supports and services.
“The launch of this platform continues the KLH-OHT’s work towards increasing navigation support and our goal for our communities is to ensure that everyone has access to supportive community resources,” says Stephanie MacLaren, Kawartha Lakes Haliburton Ontario Health Team executive director. “The new service navigation platform provides better access to those points of care and furthers a ‘no wrong door’ approach in accessing supports and services.”
“Sometimes finding and accessing the right health programs can be a challenge and you just want a voice on the phone to help you find what you are looking for,” remarks Christine Keenan, Kawartha Lakes Haliburton Ontario Health Team navigation representative. “The KLH-OHT is here to support our community by helping residents connect to local supportive programs.
